Ron was referred to the Connecting Care in the Community Program by Shoalhaven Hospital following a one-month stay, where he was being treated for Chronic Obstructive Pulmonary Disease & mild congestive Cardiac Failure.

Assessments revealed Ron was at high risk of readmission to hospital , he was having difficulty managing daily activities and there was some confusion around possible fluid restriction and medication.

“Ron and Tracey now have the tools to manage at home and are taking an active role in Ron’s ongoing health management, which is really encouraging.”
