Aboriginal Health Nutrition Services

Supporting creative and culturally responsive approaches to chronic disease management is a hallmark of GPH Aboriginal Health team. A key example of this is GPH’s ongoing commitment to deliver Specialist and Allied health outreach services for rural and remote communities. The service, established in 2012, integrates with GPH’s health screening, chronic disease management and health promotion activities.

The service brings together multidisciplinary teams of Endocrinologists, Diabetes Educators, Nurses, Aboriginal Health and Outreach Workers, Podiatrists and Dietitians to provide comprehensive, best practice diabetes care for Aboriginal and Torres Strait Islander people in their local communities. Rather than attending each individual appointment, the consumer can access all of the appointments they need in one location.
