Chronic Disease Management
To qualify for this program you must be Aboriginal or Torres Strait Islander, have at least one pre-existing chronic condition and have a GP Management Plan (GPMP) and a current 715 Aboriginal and Torres Strait Islander Health Check formulated by your GP.

What is a chronic disease?
A chronic disease is an illness that can last six months or more and has impact on your everyday life.
Example of chronic diseases include:
- diabetes (sugar levels in the blood)
- heart disease (for example high blood pressure)
- lung disease (for example asthma)
- kidney disease
- cancer

The Integrated Team Care Program is available in the Illawarra, Shoalhaven, Bega Valley, Cooma, Goulburn, Queanbeyan, Yass and ACT regions.
This program operates a Centralised Referral and Intake
Practices using Healthlink or Argus in Best Practice software should send referrals using the ITC referral template.

Referrals
To qualify for this program you must be Aboriginal or Torres Strait Islander, have at least one pre-existing chronic condition and have a GP Management Plan (GPMP) formulated by your GP. Your GP must also be signed up to PIP IHI in order to refer you.
