Community Living Support

Community Living Supports (CLS) is a free program that provides personalised support for people living with mental health challenges.

Our focus is to help you:

  • Manage day-to-day life
  • Build confidence and independence
  • Stay connected to your community
  • Work towards your personal goals

Support is flexible and designed around what matters most to you. It is culturally responsive and respectful, built around each person’s identity and community, and available for people from all backgrounds.


Who is CLS for?

CLS is for people who:

  • Are living in one of the regions below
  • Aged 16 years or over
  • Are diagnosed with a mental health condition
  • Are impacted day to day by their mental health
  • Are willing to engage with a support team

We do not require a formal diagnosis if you are Aboriginal, aged between 16–24, or a refugee or asylum seeker. Just as long as a mental health professional has decided you need extra help due to your social and emotional wellbeing.


Who will support you?

You’ll have a team who work with you, which may include:

  • Support Workers
  • Peer Workers (people with lived experience)
  • Aboriginal Wellbeing Workers
  • Care Navigators

We work together with you and, if you choose, your family, carers, friends or other supports.


How does CLS work?

Everyone’s journey is different. We work alongside you to understand your needs and what support looks like for you.

You can choose:

  • What you want help with
  • Who you work with
  • Where support happens (at home or in the community)

Support may include:

  • Building daily living skills
  • Connecting with community, work, or study
  • Strengthening relationships
  • Linking in with other services
  • Joining group activities

Support is flexible and can change over time as your needs and goals change.


How to get support

You can access CLS by:

  • Being referred by the Local Health District
  • A support service or community organisation referring you
  • Referring yourself or asking a family member or friend to help you get referred

Once we receive your referral, we’ll talk with you about your needs and what you’d like support with.
