Head to Health
Who is the service for?
Head to Health Centres provide a welcoming entry point for adults (and their support people) experiencing distress, crisis, or mental health challenges to access a range of support services.
What services are offered?
Everyone who enters Shellharbour Head to Health will receive a Wellbeing Planning Session. In this appointment, we aim to understand what is going on for you and your individual support needs. Together we will develop your Wellbeing Plan that will match you with the right support.
This could be a referral to a Shellharbour Head to Health service (including psychological intervention, peer support, drug and alcohol counselling, family and relationship support, and psychosocial groups). If there is an external service that will better meet your needs, we will link you with them.
People can self-refer to Shellharbour Head to Health by calling the Head to Health Intake Line on 1800 595 212. Alternatively, your GP or other health professionals can refer you to Shellharbour Head to Health by sending a referral letter or Mental Health Treatment Plan to firstname.lastname@example.org.
