Home Care Packages Program
What is the Home Care Packages (HCP) Program?
The Home Care Packages (HCP) Program provides government-funded support to help older Australians with complex care needs live independently at home. It offers a range of services, tailored to individual needs, and is managed through a consumer-directed approach, with four levels of packages available.

Who is it for?
To be eligible for a Home Care Package, you must be assessed through My Aged Care and be 65 years or older, or 50 years or older for Aboriginal and Torres Strait Islander people.
How can you access this program?
To access the Home Care Package program, you need to be assessed as eligible by My Aged Care.
You can apply for an assessment online through this portal or by calling My Aged Care on 1800 200 422.
If eligible, an assessor will contact you to discuss your needs and arrange an in-home assessment. Jumping into the portal, you’ll then be able to see us in the list of providers.
Going through the process for assessment can be complicated.
At Grand Pacific Health, we can help you through the process if you give us a call on 1800 315 101.
Where is this program delivered from?
We’re currently supporting people inside the ACT, Queanbeyan, Goulburn, Yass, and South West Sydney
What is the cost of this program?
The cost of a Home Care Package is a combination of government funding and fees you may be required to pay. The government provides funding based on the package level, with higher levels receiving more support. In addition to the government contribution, individuals may be required to pay a basic daily fee and an income-tested care fee, depending on their financial situation.
