‘The Way Back’ Support Service
What is The Way Back Service?
The Way Back Service is designed to support you after experiencing a tough time, such as a suicide attempt or a suicidal crisis. This program provides practical support, focusing on your well-being during this critical period when things might still feel overwhelming.
Who is The Way Back Service for?
The service is available to anyone aged 12 and older who has recently gone through a suicide attempt or crisis. If you are between 12 and 15, parental or guardian permission is required for you to join the program.
How does The Way Back Service work?
Once referred, someone will reach out to you within one business day. You’ll be connected with a support coordinator or peer worker, who will work closely with you to create a personalised plan lasting up to three months. This plan includes support and connections to community services that can help during this time. You can choose to engage with this service face-to-face, by phone, or via video call.
How can you access The Way Back Service?
Access to The Way Back Service requires a referral from hospital staff in South Western Sydney, or services connected to these hospitals, such as SafeHaven or Community Mental Health teams. These services use the Online SWSPHN Mental Health Referral Form to submit the referral.
If you are a referrer and need assistance with making a referral, please contact SWSPHN Mental Health Central Intake on 1300 797 746.
