Healthy Ageing at Home

What is the healthy ageing service?

The Healthy Ageing at Home service is an initiative of South West Sydney Primary Health Network (SWSPHN) to support older people with chronic health conditions to improve their health and wellbeing.

Grand Pacific Health will work with local communities to deliver health and wellbeing workshops to older people their families and/or carers to improve the understanding of how you can better manage your health.

Workshops

Workshops will be conducted by health professionals and tailored to meet local needs. Examples include falls prevention, managing medications, gentle exercise, how to understand and use health information to benefit your health, living well with your chronic health condition, respectful end of life care, safety in the home and where to access help when you need it.

If English is not your first language a bilingual educator will be available for the group sessions.

To be eligible to participate you must:

  • Consent to be in the program
  • Be 65 years or older, 50 if Aboriginal or Torres Strait Islander
  • Have one chronic health condition
  • Be willing to attend a series of workshops
  • Be willing to assist us to understand how we can improve health in the older population.
