Grand Pacific Health’s annual Carer and Consumer Surveys are open from today February 1, 2022 and will close on February 28. We aim to collect feedback on the overall service experience of our clients, their families, friends and other support persons. The surveys form part of our clinical governance reporting schedule and are a quality assurance measure across our organisation. The feedback also enables us to evidence what we do well and identify areas for improvement.

Surveys are available to clients and carers at each hub via both hard and soft copies. Some clients will also be sent the survey via SMS. You can also access the survey at the links below:
