Our therapists are experienced in working with people with a wide variety of problems, and are able to provide you with treatment in a flexible way to help overcome traditional barriers to treatment.

How to refer to our programs

Once a referrer has deemed a person suitable for a GPH mental health program(s),
the following region specific steps need to be completed.

If referrer is a medical practitioner, complete the following mental health treatment plan

For referrals to the Kids Count program complete the following child treatment plan

If referrer is NOT a medical practitioner, complete the following GPH mental health referral form

Send completed referral paperwork to GPH via secure fax - 02 4226 6489 or secure email.

Service area

If the referrer is a medical practitioner, complete a Mental Health Treatment Plan using practice software.

Send the completed Mental Health Treatment Plan to GPH Southern Mental Health via either:
